Renewal of “Fukushi Iryo-hi Jukyusha Sho (Welfare Medical Expenses Beneficiary Certificate)”

Notices about renewal of “Fukushi Iryo-hi Jukyusha Sho (Welfare Medical Expenses Beneficiary Certificate)” will be sent to those who have the certificate currently and also are high-school students generation, members of single-mother households, or members of single-father households.  So please take renewal procedures. 
Please note that if you do not complete the procedures by the deadline, your eligibility will be revoked and you will not be able to receive the subsidy.  If you have not received it even though you meet the following conditions, please apply for it.

Period

8:30 – 19:00, July 21st, 2021 (Wed.) – July 30th, 2021 (Fri.) except Saturday, Sunday, and National Holidays

Location

National Health Insurance Division (the 2nd counter on the 1st floor)

Eligible Persons

  • Mothers of fathers without a spouse who are rearing a child under 18 years old and whose income tax is less than 50,000 yen. (A spouse includes a person who is de facto married.)
  • Children under 18 years old who do not have parents and whose income tax is less than 50,000 yen. 

Things to bring

  • Health Insurance Card of all eligible persons  
  • Seal (Inkan)
  • Fukushi Iryo-hi Jukyusha Sho (Welfare Medical Expenses Beneficiary Certificate)
  • One of the following that shows your income tax status for the year 2020 ( Registered residents in Oizumi as of January 1st, 2021 are not required.) 
    1. Withholding certificate for CY2020
    2. Copy of Income Tax Return for  CY2020
    3. Notice on FY2021 Resident Tax Payment
    4. FY2021 Income Tax Certificate (Shotoku Kazei Shomeisho) including income deduction

High School Generation

Period

Eligible Persons

Those who meet all of the following conditions between the time of graduation from junior high school and the first march 31st after reaching 18 years old. 

  • who are being reared by parents or so on. 
  • the applicants and their dependent parents are exempt from municipal taxes. 
  • who have never been married.

Things to bring

  • Health Insurance Card of all eligible persons  
  • Seal (Inkan)
  • Fukushi Iryo-hi Jukyusha Sho (Welfare Medical Expenses Beneficiary Certificate)
  • FY2021 Resident Tax Exemption Certificate (Hikazei Shomeisho) ( Registered residents in Oizumi as of January 1st, 2021 are not required.)
