Mass vaccinations will end soon.

Vaccinations administered by the Oizumi Town will end on November 10th. 
If you wish to get vaccinated newly, please contact the call center at 0276-55-4661.
Prefectural Tomo Vaccination Center will also end on November 21st.

We have translated the documents enclosed in the envelope into Portuguese and English. So please check the following for details. 
Also, you can check several information for COVID-19 vaccination including “Translations of Pre-vaccination Screening Questionnaire for COVID-19 vaccine” in the website of Ministry of Health, Labour and Welfare. 

In addition, we explain about COVID-19 Vaccination on our bulletin “GARAPA” issued on July 15th, 2021 as attached below.   

For people who are aged between 12 and 15

For people who are aged between 12 and 15, a survey to ask if they wish to be vaccinated is conducted at first. For children attending schools in the town, e-mails have been sent to all students via their schools. For children who do not attend schools in the town and are attending foreign schools or etc., surveys have been sent by postal mail. So please check. 

When you receive the survey notice, be sure to scan the QR code on the notice or enter the URL to access the survey form on the website of the Town Hall, fill out the survey form, and click “Confirm” to complete it. Even if you do not wish to be vaccinated, please indicate so on the survey.

For people who are aged between 16 and 59

Inoculation tickets for people who are aged between 16 and 59 were sent out on July 13th (Tue.).  Due to the large number of people, please understand that it takes 1-2 weeks for delivery. At first, we would like to conduct a survey asking you if you have any underlying medical conditions and if you wish to be vaccinated at vaccination sites administered by the town.  

If you wish to be vaccinated at the sites administered by the town, please fill out the following information in the survey form, and post it by using the self-addressed envelope enclosed.  

  1. Names of all persons who wish to be vaccinated.
  2. Date of birth of all persons who wish to be vaccinated.
  3. Phone number
  4. The number on the inoculation ticket of all persons who wish to be vaccinated. (10 digits number)
  5. Occupation
  6. Presence of underlying disease (Please circle either “Yes” or “No”.)

Inoculation tickets for people who are aged between 60 and 64 were sent on June 30th (Wed.). 
The vaccination dates and the location were set by the town.

Vaccination for COVID-19

Eligible persons to be vaccinated and order of inoculation

The order of inoculation is decided by the government. Current schedule is as follows. 

  1. Health Care Workers
  2. The elderly (who were born before April 1st, 1957)
  3. Those who have underlying diseases other than the elderly, those who are working in facilities for the elderly. 
  4. Those who are other than 1 to 3 mentioned above.

Consent is required for vaccination. 

Vaccination will only be administered with consent of the person to be vaccinated, after sufficient information has been provided.
People who receive the vaccination will do so by their own volition, with understanding of both the efficacy of the vaccine in preventing infectious diseases and the risk of adverse reactions. No vaccination will be administered without the consent of the recipient.

Vaccination frequency

Pfizer Vaccine: 2 dose series (the same vaccine) with a 3 weeks  interval 
Moderna Vaccine: 2 dose series (the same vaccine) with a 4 weeks  interval 

The timing of vaccination

For the elderly (who were born before April 1st, 1957)

Vaccination Schedule: Vaccinations are scheduled to be completed by the end of July.

For people who are aged between 60 and 64

Vaccination Schedule: Vaccinations are scheduled to start from the end of July and to be completed by early September.

For people who are 59 years old or younger

Vaccination Schedule: Not fixed yet.

  • Preliminary Examination Form (Yoshin-Hyo) for COVID-19 Vaccination
  • Identification Documents (Driver’s License Card, Zairyu Card, etc.

Other Notes

  • Please measure your body temperature at your home in advance on the day of vaccination. If you have an obvious fever (normally 37.5 degrees Celsius) or are feeling unwell, please refrain vaccination and contact the call center to cancel. 
  • If you live outside of the Oizumi Town due to hospitalized, admission, or other unavoidable reasons, and will be vaccinated outside of the of Oizumi Town, please inquire of the local government where you will be vaccinated. (“Jyushochi Gai Sesshu Hyo ( Notification of vaccinations at addresses other than the registered address) is required.)

If you have lost or broken the inoculation coupon tickets, we will reissue them for you. 

How to apply for the reissue of coupon tickets 

  • By Phone
    Please call to the Health Promotion Division (Kenko- Zukuri-ka). After asking you the situation, we will reissue. 
  • Application at the counter
    Please submit “Sesshu-Ken Saihakko Shinseisho (Request Form to Reissue Inoculation Coupon Tickets)” to the Health Promotion Division (Kenko-Zukuri-ka). After checking the form, we will reissue. 
  • By Postal Mail
    Please fill out required information in “Sesshu-Ken Saihakko Shinseisho (Request Form to Reissue Inoculation Coupon Tickets)” and send it with broken inoculation coupon tickets (if you have) to the following address by postal mail. After checking the form, we will reissue.
