An application form for the “Individual Number Card (My Number Card)” that a QR code was printed on is sent to those who do not have the card from the “Japan Agency for Local Authority Information Systems (J-LIS)” from January, 2021 to March.
By using this application form, you can apply for your “My Number Card” by postal mail or by on-line.
Addressee
Those who have not yet applied for issuance of their “My Number Card” even though they have not yet obtained the card. But following persons are not eligible.
- Those who are 75 years old or older as of October 31st, 2020. (Persons above will be notified when they renew their insurance card of the “medical care system for elderly aged 75 and over (Koki-Koreisha-Iryo-Seido).”)
- Those who have received the application form with the QR code in 2020 due to birth or moving-in from overseas.
- Foreign residents with a fixed period of stay (Information will be provided when the period of stay is renewed.)
- Due to unavoidable reasons, those who have registered a place other than the address on their resident card as their residence information in the Basic Resident Registration System.
