Oizumi Town Subsidy Project for Inspection Fee of COVID-19
Applicable Inspection
PCR test, Antigen test
Eligible Person
Persons who meet the following conditions.
- Registered residents in the town as of the day that they received the PCR test or the Antigen test by instruction of their doctor.
- Persons who receive a doctor’s consultation on October 1st, 2020 or after.
Expenses to be subsidized
Self payment portion of the doctor’s consultation fee at your doctor when you were required to receive the test. (excluding medicine or injection)
Subsidy
Total amount of expenses to be subsidized, but up to 5,000 yen. (One time only per person)
Required Documents for Application
- Application form to grant the subsidy for the inspection of New Coronavirus Infection provided by Oizumi Town (Oizumi-machi Shingata Coronavirus Kansensho Kennsa Jyoseikin Kofu Shinseisho)
- A copy of the receipt of both the inspection and the consultation fee
- A copy of the detailed statement of both the inspection and the consultation fee
