Those who are 59 years old or younger and wish to be vaccinated with COVID-19

If you are 59 years old or younger, and have not applied for the COVID-19 vaccine, and now would like to be vaccinated, please contact the Health Promotion Division.

To make an appointment for vaccination, we need your name, date of birth, and vaccination ticket number. Please have your vaccination ticket ready at hand.
If you have lost your vaccination ticket, please ask us to reissue it.

  • Those who have returned the vaccination request survey by QR code or by mail will be mailed a notification of the vaccination date. So please wait for a while.
  • If you wish to be vaccinated, please return the vaccination request survey form enclosed with the vaccination coupon in a self-addressed envelope by September 30th, 2021, or bring it directly to the Oizumi Town Health & Welfare General Center. You can also call the center by phone.
  • If you wish to be vaccinated as soon as possible, please consider using large scale vaccination centers administered by the prefecture or the national government.  The vaccine currently being used at the large scale vaccination centers is the Moderna vaccine (for people 18 years of age or older on the date of vaccination).

Free of charge.

Frequency and interval of vaccination

To get the full effect of the vaccine, two doses of the same vaccine should be given. In addition, the interval between the first and second vaccination should be 20 days. If you receive a different type of vaccination before or after COVID-19 vaccine, please make an interval of at least 13 days.
