The town subsidizes the purchase of medical wigs and chest prosthetic devices for cancer patients suffering from hair loss and changes in appearance due to cancer treatment.
Eligible Persons
Cancer patients who meet the following conditions.
- Those who need medical wigs and so on against hair loss and changes in appearance due to cancer treatment.
- Those who are registered as a resident in Oizumi Town at the time of the application.
- Those who have not received the same kind of subsidy from other municipalities in the past.
- Those who have not failed to pay municipal taxes.
Subsidy
- Medical Wigs (Wig, Net) … up to 30,000 yen
- Chest Prosthetic Devices (Brassieres, Pads, Artificial Breast) …up to 20,000 yen
Each person can apply once for each item. (up to 50,000 yen)
If you purchase both items at the same time, you can apply for them together at a time.
Application Method
Please apply directly to the Health Promotion Division with the required documents after purchasing the applicable product.
Required Documents
- An application form and request form for subsidy for purchase of medical wigs, etc. for cancer patients provided by Oizumi Town
- Documents proving receipt of cancer treatment (copy of medical certificate, etc.)
- A copy of the receipt for the purchase of medical wigs, etc. (One year has not passed since the date of issue.)
- Your identification documents (Driver’s license, etc.)
- Something that shows the payee’s bank account (bankbook, etc.)
Application Period
Within one year after the purchase (Issue date of the receipt)
Please note that purchases on or after April 1st, 2021 are eligible.
