Renewal of “Fukushi Iryo-hi Jukyu Shikakusha Sho (Welfare Medical Expenses Beneficiary Certificate)” for Children

The “Fukushi Iryo-hi Jukyu Shikakusha (Welfare Medical Expenses Beneficiary Certificate)” for children who were born from April 2nd, 2015 to April 1st, 2016 will be renewed. As the existing welfare medical expense beneficiary certificates will expire on March 31st, 2022, a new certificate will be mailed to the eligible children. Please dispose of your expired certificates in a way that does not reveal your personal information, or return them to the National Health Insurance Division (Kokumin Kenko Hoken-ka) at the 2nd counter on the 1st floor of the town hall.
Please note that children in single-mother families and single-father families who are eligible for the benefit are not eligible for this renewal, even if they are of the eligible age. 

Requests to those who use Welfare Medical System  

If you are eligible to use the “Kohi Futan Iryo Seido (Publicly Funded Health Care Systems)” such as the Pediatric Chronic Specific Disease Medical Care, Japan Sports Promotion Center Disaster Mutual Aid Benefits, etc. other than the Welfare Medical System, those systems will be applied preferentially. In order to ensure the stable operation of the Welfare Medical System, we ask for your understanding and cooperation in using it conjunction with other Publicly Funded Health Care Systems.
