Partial subsidies for enrollment in nursing school and other schools

In order to enhance local medical care, the town will subsidize a portion of the entrance fee for new students or their guardians at schools that train public health nurses, midwives, nurses, and assistant nurses.

Application Period

Within 6 months from the date of enrollment in school, etc, excluding Saturdays, Sundays, and national holidays

Eligibility

Those who entered a school designated under the Act on Public Health Nurse, Midwife, and Nurse or a public health nurse training school, midwife training school, nurse training school, or assistant nurse training school in 2023 and have an address in the town at the time of admission.
Or, those who have moved out of town to commute to school.
However, if a member of the household to which the subsidy recipient belongs is in arrears of town taxes, he/she is not eligible for the subsidy.

Subsidy Amount

The amount equivalent to one-half of the entrance fee paid to the school, etc., up to 100,000 yen (any fraction less than 1,000 yen will be rounded down). 
However, the subsidy may be returned if the applicant withdraws from the program or fails to qualify for the subsidy after FY2019.

How to apply

Please apply to the Health Promotion Division with the application form and necessary documents.
If you have withdrawn from school, please submit a Report of Withdrawal from Oizumi Town Nursing Admission Fee Subsidy to the Health Promotion Division within one month of your withdrawal.

Documents required for application

1.  Oizumi Town Nursing Staff Entrance Fee Grant Application Form
2.  Entrance fee receipt or document certifying the amount paid as the entrance fee
3. Documents certifying that the applicant is enrolled in a school, etc.
4. Documents proving that no member of the household to which the subsidy
     recipient belongs is in arrears of town taxes
5.  A copy of the certificate of residence, a copy of the certificate of removal of the           certificate of residence, or a copy of the attached copy of the family register of 
  the person eligible for the subsidy
6. Documents that confirm the payee’s bank account  (a copy of the account of                 financial institution, the name of the payee, etc.)

Please note that documents 4 and 5 are not required if you submit a consent form for the town tax survey.
