Obtaining Certificates with Oizumi-machi Official LINE

The following certificates can be obtained from the town’s official LINE account using your My Number Card.
There are options to receive the certificate by mail to your address or pick it up at the counter of the Resident Affairs Division.  Please take advantage of this service.

Certificates for which you can apply

Copy of residence certificate (for all or part of household)

  • 300 yen per application
  • Applicants must be a person with an address in Oizumi-machi and a member of the same household.
  • Removal forms cannot be obtained.

Seal registration certificate

  • 300 yen per application
  • Person who can apply is the person whose seal is registered in Oizumi-machi.

Official copy of family register (All/A Part)

  • 300 yen per application
  • Applicants must have their domicile of residence in Oizumi Town and members of the same family register
  • Deleted/Past Residential Record in Family Register cannot be obtained
  • Certificate of family register (complete and individual) (copy of family register) 450 yen per application
  • Person whose permanent domicile is in Oizumi Town and those who are registered in the same family register can be obtained.
  • (A certificate of removal from the family register (total and personal matters) cannot be obtained.

Income certificate

  • 300 yen per application
  • The applicant who resided in Oizumi-machi on January 1 of the year to which the first day of the fiscal year (April 1) of the certificate being applied for belongs.
  • The last five fiscal years, including the most recent fiscal year, are available.

Proof of Taxable Income

  • 300 yen per application
  • The applicant who resided in Oizumi-machi on January 1 of the year to which the first day of the fiscal year (April 1) of the certificate being applied for belongs.
  • The last five fiscal years, including the most recent fiscal year, are available.

Taxation (Tax exemption) certificate 

  • 300 yen per application
  • The applicant who resided in Oizumi-machi on January 1 of the year to which the first day of the fiscal year (April 1) of the certificate being applied for belongs.
  • The last five fiscal years, including the most recent fiscal year, are available

Municipal & Prefectural Residence Taxes certificate

  • 300 yen per application
  • The applicant who resided in Oizumi-machi on January 1 of the year to which the first day of the fiscal year (April 1) of the certificate being applied for belongs.
  • 申請に必要なものYou may acquire the last five fiscal years, including the most recent fiscal year, but not if the required fiscal year is tax-exempt.

  • PIN (6- to 16-digit alphanumeric password) for the digital certificate for signature

Identification documents

Required for receiving at the counter of the Town hall.

Notes.

  • Payment is only accepted with LINE Pay.
  • The mailing address must be the current address registered on the applicant’s My Number Card.
  • Residence certificates and seal registration certificates with my number will be sent by registered mail.
  • Postage costs will be borne by the applicant.
  • If the postage is insufficient, we will send it to you on a “payee for shortage” basis.
  • We will contact you on line or by phone if your application is incomplete or unclear.
  • If there are no deficiencies in the application, applications received by 4:00 p.m. on the same day will be delivered by mail or over the counter on the next open day.
  • If you choose to receive the application at the counter, you will need to present your My Number Card or other identification documents at the counter.
  • Since the application cannot be cancelled after submitted, please confirm the contents of the certification before submitting the application.

Handling of Personal information

  1. Oizumi-machi handles personal information appropriately in accordance with the Act on the Protection of Personal Information and other related laws and regulations.
  2. Oizumi-machi collects personal information such as names submitted as data for applications and notifications.  The collected personal information will be used only for the purpose of receiving and accepting applications and sending certificates.
  3. Personal information collected will be used only for the purpose of receiving and accepting applications and sending certificates.
  4. Oizumi-machi performs the confirmation authentication of the user’s identity using the digital certificate for signature stored in the My Number Card.  It also reads the user information stored in the My Number Card’s Ticket Information Input Aid AP and provides it to the requesting user, etc.
  5. Oizumi-machi will use the certification service information (refers to the certification service information stipulated in Article 44 of the Act on Certification Service of Local Public Organization Information System Organization Pertaining to Electronic Signatures, etc. (Act No. 153 of 2002)) to confirm the validity of electronic certificates for signatures.
