Partial subsidy for Pneumococcal vaccination fee for the elderly

The town partially subsidizes the cost of pneumococcal vaccination for the elderly in order to prevent pneumonia and serious illness among the elderly.

Eligible persons

Persons who meet all of the following conditions

  • Persons who have an address in the town and are 66 years of age or older on the day of vaccination
  • Persons who have not received a pneumococcal vaccination in the past

Subsidy amount

2,000 yen

The subsidy is limited to one per person. If the cost of the vaccination is less than the grant amount, the cost of the vaccination will be used as the grant amount.
In addition, persons in households receiving public assistance are subsidized for the full cost of vaccinations.

Application Method

  1. Please consult with the Health Promotion Division about the subsidy program prior to vaccination.
  2. Those who have received an explanation of the subsidy system and agree to it are required to receive the vaccination at a medical institution and pay the full amount of the vaccination fee.
  3. After the vaccination, please apply at the Health Promotion Division counter or through  LINE account with the following required documents.
Documents required for application
  • Oizumi-machi Pneumococcal Vaccination Subsidy for the Elderly Application and Claim Form (not required if applying through LINE)
  • Copies of medical receipts
    However, it must include the name of the person who received the vaccination, the type of vaccine, the name of the medical institution, the date of vaccination, and the cost of vaccination.
  • Bank transfer account for grants

Application deadline

Within 6 months from the date of vaccination or by March 31, 2025, whichever comes first.

Notes.

Before receiving the vaccination, be sure to visit the Health Promotion Division for an explanation.
Applications are accepted from 8:30 am to 5:15 pm. However, Saturday, Sunday, national holidays, and the year-end and New Year holidays are excluded.
